Sign-up/Change Member Information

Download the MAPS Membership Form and follow the instructions below:

1. Read the Articles of Agreement if you are a new member.
2. Complete the highlighted blocks LEGIBLY and use your full LEGAL name.
3. Cut out the form and make a copy for yourself and for each beneficiary.
4. Give the form to your MAPS representative who will fill in the REP NO. Please use the rep listing document attached to the membership form to help identify the representative in your area.
5. Your MAPS representative will forward your form to the MAPS Bookkeeper.
